Medical examinations required by U.S. Immigration Service.

For your first appointment, we require you to bring a photo ID, your vaccination records translated into English, and your full payment of $200 for this process. The $200 covers the initial nurse visit, TB test, the physical examination, as well as our service for applying vaccination and drawing blood (the vaccination and bloodwork themselves are not included). Insurance doesn’t cover the $200 fee. Please go to http://www.uscis.gov/files/form/i-693.pdf and print out this required form and bring it with you for your visit; otherwise, we can print it out for you in our office at no additional charge. Remember, the vaccines and tests required by immigration are not included in the $200 charge. After your first visit you will know what extra vaccines or test you may require.Please call your health insurer to see if these tests and vaccines are covered; if you lack insurance, you will be responsible for the full cost. We strongly encourage you to obtain your vaccination records from your country; it may save you some money. We may be able to give some of the vaccinations and some of the tests on site. But you may need to go elsewhere, if needed. Also remember, this is not a one-step process, you may need at least 3 visits. The length of the process may depend on your health, and your individual circumstances.
